Attention 11th grade parents and guardians, 

There are immunization requirements for entry to the 12th grade. In accordance with Ohio Revised Code 3313.671, all students entering the 12th must have 1 or 2 doses of the MCV4 (meningococcal conjugate vaccine serogroups A, C, W and Y) prior to entry into 12th grade. This also applies to students attending CTC. The first dose of MCV4 is typically given between the ages of 11 and 12 with a second dose given between 16 and 18. If your student had one dose before 16, they will be required to have an additional dose of MCV4. If your student has not had a previous dose of MCV4 and is now 16 to 18, they will only be required to have one dose. Meningococcal B vaccine is not currently required. Please check with your healthcare provider to verify the status of your student’s vaccines. 

If you have any questions regarding immunizations, please contact your healthcare provider, the Clark County Combined Health District, or the Northwestern Clinic. An updated immunization record must be provided to the school on entry to 12th grade. Failure to provide documentation of these vaccines by the 15th day after the start of 12th grade (September 3, 2025) will result in exclusion until the requirement is met.
